Referral Process for Audiological Evaluation

Audiological Evaluation is available through the District audiologist. Students who do not pass a hearing screening performed by the school Speech-Language Pathologist may be referred. If you have concerns regarding a student's hearing, please request a screening from your school SLP.  When necessary, the SLP may request screening support from the audiologist to ensure the screening is completed. 

To refer a student for testing, obtain SIGNED parent consent using the appropriate form. For students on an IEP, or being considered for an IEP, please use the consent for evaluation in EdPlan. For students on a 504, please generate a permission form in Infinite Campus under the PLP section. For all other students, please Generate A Permission Slip by entering student data clicking submit, and retrieving the permission slip from your email.  Once the permission slip is signed,  send the SIGNED consent (ALL PAGES) to the audiologist at Amity Elementary.  Parents will be emailed or called to schedule an appointment. Results will be discussed with the parent(s) at the evaluation and a report will be sent to the parent(s); school teams will receive a copy of the report. The audiologist will submit the original report to the DSC.  For students on educational plans, e.g. IEPs or 504s, those teams will need to ensure results are reviewed and a Written Notice is sent to parents. 


*Please refrain from stating “hearing impairment” and use “hearing ability” or "hearing needs" to be most sensitive to the Deaf community. If referring to the child, please use “deaf or hard of hearing” rather than “hearing impaired.”
